Terms & Conditions: I certify that I am at least 18 years of age. I understand that enrollment options and prices shown do NOT include shipping and handling. I understand real-time shipping charges may apply; applicable sales tax WILL apply based on location. Enrollment forms for continental US, AK or HI residences only. I understand all final charges are determined when the order is processed and may increase based upon assessed shipping and taxes when applicable. Shipping rates will be based on distance from Gano Excel USA, Chino CA. to the point of destination using current real-time FedEx Rates; according to the service provided (Priority, 2nd Day, Normal, ect.) Shipments to PO Boxes, AK and HI could be based on current USPS rates. All Applicable taxes will be assessed. I understand that by signing this form, I authorize Gano Excel to substitute the closest available product to automatically maintain my monthly qualification, as stated on the reverse side (page 2) of this form as it relates to my Auto-Ship Program.
By signing, I acknowledge that I have carefully read all of the Terms & Conditions on this application. I understand the Terms & Conditions contained herewith and agree to be bound by them. I also understand that I may terminate my independent Affiliate status at any time with written notice to Gano Excel.

Términos y Condiciones: Certifico que tengo 18 años de edad. Entiendo que todas las cargas finales se determinan cuando se procesa la orden y pueden aumentar basado en el tipo de envío e impuestos aplicables. Las tarifas del envío serán basadas sobre el peso, volumen y distancia usando las tarifas en tiempo real actuales de FedEx; según el servicio proporcionado (prioridad, 2do-Día, normal, etc.); Los envíos a las cajas de PO, a AK y al HI se podían basar en tarifas actuales de USPS. Todos los impuestos aplicables serán determinados. (Si los artículos que usted quisiera incluir para el Auto-envío no se enumeran en esta forma, comunicase con nuestro Departamento de Servicios al Cliente al 626-338-8081 para comprobar si hay disponibilidad de estos artículos.) Entiendo que con firmar esta forma, yo autorizo que Gano Excel sustituya el producto mas similar disponible, por un producto dejado pendiente, para poder mantener mi cualificación automáticamente.
Al firmar, reconozco que he leído cuidadosamente todos los Términos y Condiciones de ambos lados de esta solicitud. Yo entiendo los términos y condiciones contenidos en este documento y de acuerdo en estar obligado por ellos. También entiendo que puedo terminar mi estado afiliado independiente en cualquier momento con aviso por escrito a Gano Excel.
